Use this form to notify the department of any change in contact details.

Request for water entitlement data attached to land (PDF, 92K)*

This form is used by individuals or entities (e.g. solicitors) acting on behalf of an individual to request information relating to water licences, interim water allocations and associated information or approvals that are attached to a property.

Stock or domestic allowance notification form (PDF, 176K)* 

Use this form to notify the department of a claim for a stock or domestic allowance.

Water meter reading notification form (PDF, 60K)* 

This notification form is used to provide water meter readings to the department.

Examples when a meter reading may be required include:

  • at the sale of the property
  • special end of water year reading
  • dealings which involve a tradable water allocation (sale or transfer)
  • other special meter reading events.

In the event of a property sale this information could be used to apportion water usage charges.  Apportionment of fixed (Part A or allocation charges) will remain the responsibility of the transferor and transferee.

Water use assessment report (PDF, 72K)*

Water sharing rules are the rules prescribed under the Water Regulation 2002 (Qld) for a water licence not managed under a resource operations plan. The water sharing rules describe the management arrangements for access to water within a water management area

If water sharing rules apply, meter malfunction or breakdown must be reported to the department as soon as possible so that arrangements can be made to repair or replace the water meter.

During the period between reporting the incident and the repair/replacement of the meter, the water entitlement holder must complete W2F095 Water use assessment report (PDF, 72K)* to account for water use during this time.

Resource operations licence / distribution operations licence

Use this form to apply for amalgamating a resource operations licence with another resource operations licence in the same water supply scheme, into one single licence.

Development permit (operational works related to water)

Request for ‘Consent of the chief executive’ for new operational works that take water or interfere with the flow of water (PDF, 73K)*

Use this form to comply with the requirements under section 260 of the Sustainable Planning Act 2009 (Qld). Before you submit an application for a development approval, you are required to obtain evidence that the chief executive of the Department of Environment and Resource Management (referred to as Consent of the chief executive) is satisfied that the proposed water related development is:

  • either consistent with an entitlement to the resource (a water authorisation such as a water licence, an interim water allocation, a water allocation, a water permit or a seasonal water assignment)
  • or in some instances an entitlement to the resource (a water authorisation) may not be required and the development application can proceed in the absence of an entitlement to the resource.

Note: This request for Consent of the chief executive is not an application for a development approval.

Request to extend, change or cancel a development permit for operational works related to water (PDF, 108K)*

This form must be used for a request to the Department of Environment and Resource Management (DERM), as the responsible entity, for any of the following types of request made under the Sustainable Planning Act 2009 (Qld):

  1. an extension of the period before the development permit lapses where DERM is the assessment manager (section 383)
  2. to make a permissible change to a condition of the development permit where DERM imposed the condition as the assessment manager or concurrency agency (section 369)
  3. to make a permissible change to the development permit, other than a change to a condition, where DERM is the assessment manager (section 369).

Request for allowance of volume of water taken by a constructing authority (PDF, 68K)*

This form is to be used by a constructing authority and a holder of a metered entitlement to request an allowance for the volume of water taken under section 20(8) of the Water Act through a meter that is used to take water under a metered entitlement administered by the department.

A constructing authority as defined under section 2 of the Acquisition of Land Act 1967 and the Protocol - Authorised taking of water without a water entitlement under the Water Regulation 2002, may take water with the need for a water permit or a water licence for the purpose of construction or maintenance of infrastructure (that the constructing authority may lawfully construct or maintain) provided the water is taken in accordance with:
